Part of the Lyfechannel Integrated Health Series for Samsung: Better Connections for Better Outcomes

The Lyfechannel Integrated Health Series for Samsung brings together great patient programs, parallel support team experiences and care provider connectivity to help patients build basic habits in diet, physical activity and drug adherence.

The PATHWay Champion experience is for supporters of patients on a PATHWay diabetes journey, and makes supporters an integral part of the patient progress. The Gear 2 experience gives supporters an instant window into their patient/friend's progress in the program, recent glucose measures and adherence patterns. The Gear 2 also has education components to help the supporter understand Diabetes better and be a better supporter, and the ability to directly send support messages to the PATHWay partner. The Gear 2 experience pairs with an Android app that has richer media, more information and the ability to manage data sharing permissions.

PATHWay is a program for patients new to Diabetes taking their first steps in a long lifetime of thriving through this health journey. Lifestyle change is difficult, and it is hard to tap into the daily motivation to make these changes. The combination of the Samsung Galaxy app paired with the Gear 2 experience makes this journey easier and puts triggers into place to encourage the development of new habits. For patients whose physician is on the largest EHR in the country, the Galaxy/Gear experience can be connected with their care provider to create a better “continuity of care”.

The PATHWay Gear 2 experience supports the development of a critical early-stage behavior in life with Diabetes — the recording of glucose readings, physical activity and carb counts. Additionally, 30 second learning vignettes educate and empower the person new to Diabetes to take control of their condition.

The PATHWay app for the Samsung Galaxy provides a richer, more engaging experience, with greater adherence tools and controls for sharing experiences with supporters and care providers.

For Phase I of the Samsung Gear App Challenge we submitted an app to pair with the Gear 2 experience to simulate a “mid-program” data set. Health programs become more interesting as they progress. so for this initial evaluation of the Gear 2 experience the Samsung App will make the Gear 2 “act” like like it would after registration, initial data collection and other early experience requirements that would require more judge interactivity than we would most likely be allotted at this phase. This app will show you the data exchange with the Gear 2 and the Samsung Galaxy that would usually be “behind the scenes”. If we progress to the next round in the Challenge we will pair the Gear 2 experience with the full PATHWay Champion app and instructions for account creation and initial patient profile development.
